Privacy Policy
The MyChildCMC App is intended for use only by individual participants in a clinical research study. As a condition of participating in that study, participants agree to share certain personal information for use in the study. The MyChildCMC App enables participants to share information as described in the consent and authorization document that applies to that study, which may be found here: Consent Form. The University of Utah will use information obtained through the app only as described in that consent and authorization document, or as you may otherwise agree in writing.
Terms & Conditions
To use the MyChildCMC App, you must be a participant in a clinical research study (the "Study") conducted by the University of Utah ("University") or an administrator, study coordinator, or other individual specifically authorized by the University to use the app (an "Authorized User"). If you are an Authorized User, the University will provide you with credentials, which you agree to safeguard and not share, and information necessary to create an account for using the MyChildCMC App in connection with the Study. The MyChildCMC App, together with the functionality for submitting information and the servers and related computer hardware used by the University for storing, processing, and accessing such information are collectively referred to herein as the "MyChildCMC Platform."
As a condition of using the MyChildCMC App, you agree not to use any aspect of the MyChildCMC Platform for any purpose that is unlawful or prohibited by these terms of service, or any other purpose not reasonably intended as a component of your participation or support of the Study. By way of example, you agree not to use the MyChildCMC Platform:

To attempt to create an account or otherwise upload or access information via the MyChildCMCApp if you are not an Authorized User, to create more than one account, or to use the MyChildCMC App to view the information of another user whose information you are not authorized to view;
- To abuse, threaten, harass, or intimidate any person;
- For any purpose that is not permitted by the laws of the jurisdiction where you use the MyChildCMC App;
- To submit false information to the MyChildCMC Platform;
- To use any automated means or computer program to upload information to the MyChildCMC Platform;
- To access or attempt to access the source code for the MyChildCMC App or to copy, reverse engineer, decompile, or otherwise access the MyChildCMC App for any purpose other than facilitating your participation in the Study, or to attempt to violate the intellectual property rights of the University or any third party.
You acknowledge and agree that the MyChildCMC Platform is made accessible solely as a means of supporting the Study and that the University of Utah may lock your account or otherwise terminate your ability to share information via the MyChildCMC App or to access information stored on the MyChildCMC Platform at any time, for any reason.
You acknowledge and agree that the MyChildCMC Platform is an experimental platform being studied as part of clinical research, and that its functionality is limited to displaying, transmitting, and providing information. The MyChildCMC Platform is not a substitute for professional healthcare and should not relied on as such. To the maximum extent permitted by applicable law, if you are not an Authorized User, the University disclaims any and all liability for any harms arising out of your use of the MyChildCMC App and, by downloading the MyChildCMC App, you hereby waive any claims you may have against the University related to such harms.
